Military Targeted Mental Health Case Management
We provide case management services to adults living with serious substance use issues, mental illness, or those with a dual diagnosis of substance use and mental illness. The primary goal of mental health targeted case management is to connect clients with needed services, including medical, social, educational, mental health, housing, insurance, employment, financial, and other services, so they can function as independently as possible in our communities.
Golden Years Case Management Program
This program provides services to individuals who have been determined to have a medically fragile condition that results in prolonged dependency on medical care for which daily skilled intervention is necessary and or a developmental disability or risk for developmental disability. We provide support to families by assisting individuals to remain cared for in their homes. Our case managers use a family-centered approach to care, and through collaborative partnerships, act as advocates, resource persons, facilitators, and paperwork coordinators for clients and their families.
Transition Case Management
Transitional planning is defined as a dynamic, interactive, collaborative, and interdisciplinary process of assessment and evaluation of the healthcare needs of patients and their families or caregivers during and after a phase/episode of illness. Our Case Managers plan and broker necessary services and resources (e.g., durable medical equipment) identified based on the patient’s condition. In addition, we ensure that these services are delivered in the patient’s next level of care (i.e., setting) or after discharge from a hospital. This process is systematic and aims to facilitate the transition of patients from one level of care to another more appropriate, necessary, and reimbursable level without compromising the quality and continuity of care, or the services being provided.
Case Management for Adults, Adolescents & Children
Our team will provide a direct interface with our clients and will work closely with their physicians to coordinate care and services for high-risk waiver participants. The goal of complex case management is to help clients regain optimum health or improved functional capability, educate members regarding their chronic condition, and reinforce the primary care physician treatment plan. Enrollment criteria for complex case management include: major organ transplant, major trauma, four or more chronic conditions, three or more admissions within a 12 month period, polypharmacy utilization consisting of more than 30 prescriptions per quarter, or cancer diagnosis requiring multiple treatment modalities with complex care coordination across multiple specialties
